About Our Membership Plan

Our practice operates on a membership-based care model designed to support whole-person health — physical, mental, emotional, and spiritual. This approach allows us to move beyond rushed, visit-by-visit care and instead provide thoughtful, relationship-centered support tailored to each patient.

Beginning January 1, 2026, all patients will be cared for under this membership model, regardless of insurance status.

What the Membership Provides

Our membership supports the non-visit-based care that traditional insurance does not cover, including:

Extended and comprehensive visits
Secure messaging and between-visit support
Home healthcare and telehealth as clinically appropriate
Wellness, lifestyle, and spiritually grounded coaching
Proactive mental health check-ins
Navigation of care coordination, referrals, and follow-up

This structure allows us to provide consistent access, deeper care, and a more personalized experience.

🌿 Wellness Passport: Our First Community Partnership

As part of your Sequence To Health membership, we are proud to partner with Float and Salt Healing Center to provide members with monthly opportunities to support relaxation, recovery, and overall wellness.

Eligible members may choose from the following wellness services each month:

Float Therapy
Infrared Sauna
Himalayan Salt Room
Oxygen Bar
Herbal Foot Massage
Chiropractic Care
Classic Head Spa
Korean Express Facial
Body Massage

This partnership is part of our commitment to supporting the whole person—mind, body, and overall wellness.

One eligible wellness service may be selected each month. Membership must be active and in good standing, and members must remain actively engaged in their healthcare journey to participate.

Service availability, scheduling, and specific service details are subject to Float and Salt Healing Center's policies. Additional services, upgrades, retail purchases, and gratuities are the responsibility of the member.

Insurance & Membership

Why We Use a Membership Model

Membership fees are separate from insurance billing.

Patients with insurance will still have their insurance billed for medical services such as office visits, labs, and medications.

Your membership fees will be alloted to any copays, co-insurances, or deductibles you may have ensuring predictable pricing and no pop up fees.

$75.00 one time initiation fee per person

$120.00 per month for the first 3 people

$50.00 per month per additional family member

Family Without Insurance

$100.00 one time initiation fee per person

$165.00 per month for the first 3 people

$60.00 per month per additional family member

Hybrid Family Membership ( mixed-insurance and no insurance families)

$100.00 one time initiation fee per person

$150.00 per month for the first 3 people

$55.00 per month per additional family member

This membership is not required but is recommended for those with Hip Nation/Sedera Health Insurance or Medicare or Medicaid

Wellness Services

We offer a wide range of wellness services to help you achieve optimal health. From psychiatric counseling, nutrition counseling to stress management, we have you covered.

Primary Care

We provide comprehensive care for ages 7 and older. We focus on building relationships with our patients to provide personalized care.

Preventive Care

We believe that prevention is key to maintaining good health. Our preventive care services include annual physicals, prescriptions for immunizations, and cancer screenings. Our Wellness Passport extends preventative wellness beyond the clinic by connecting members with community partners such as Float and Salt Healing Center, offering additional opportunities to support physical, mental, and emotional well-being. As our membership grows, so will our network of wellness partnerships.

Telemedicine

We offer telemedicine visits for your convenience. You can connect from the comfort of your own home or office.

Is the membership required?

Yes! For those with commercial insurance or no insurance! Our practice operates on a membership-based care model to allow for personalized, high-quality care and improved access.

If I have insurance, why do I have to sign up for a membership?

We still accept insurance and will bill your insurance for office visits; however, the membership fee covers non-insurance services, including:

Care coordination
Enhanced access to the provider outside of scheduled visits
Multiple services in one visit (i.e. both primary care and mental health)
Wellness and spiritual coaching
Administrative and communication support
Extended visit availability
Home health and telehealth as needed
Access to our community partners

If I have insurance, will I still be billed for copays, co-insurances, or deductibles on top of the membership fee?

No! For those with insurance, part of your monthly membership fee will go towards any copays, co-insurances, or deductibles you may have. This ensures you have simple, predictable payments for your healthcare.

When will the membership be billed?

All monthly membership fees will be automatically billed on the 5th of the month, regardless of when the initiation fee was paid.

Can I cancel my membership later?

Memberships may be cancelled once you have signed the membership cancelation form we can provide for you, but initiation fees are non-refundable.
