Our practice operates on a membership-based care model designed to support whole-person health — physical, mental, emotional, and spiritual. This approach allows us to move beyond rushed, visit-by-visit care and instead provide thoughtful, relationship-centered support tailored to each patient.

Yes! This membership is required for ALL patients who want to be seen at Sequence To Health. Our practice operates on a membership-based care model to allow for personalized, high-quality care and improved access.

We still accept insurance and will bill your insurance for office visits; however, membership fees cover separate fees from insurance and are required for enrollment in the practice.
No! For those with insurance, part of your monthly membership fee will go towards any copays, co-insurances, or deductibles you may have. This ensures you have simple, predictable payments for your healthcare.
All monthly membership fees will be automatically billed on the 5th of the month, regardless of when the initiation fee was paid.
Memberships may be cancelled with written notice per our policy, but initiation fees are non-refundable.
