Introducing Our New Membership-Based Care Model!

Beginning January 1, 2026, all patients will be required to enroll in our membership model, designed to support consistent, relationship-based care, independent of insurance status. Please see below for pricing and more information.

About Our Membership Plan

Our practice operates on a membership-based care model designed to support whole-person health — physical, mental, emotional, and spiritual. This approach allows us to move beyond rushed, visit-by-visit care and instead provide thoughtful, relationship-centered support tailored to each patient.


Beginning January 1, 2026, all patients will be cared for under this membership model, regardless of insurance status.

What the Membership Provides

Our membership supports the non-visit-based care that traditional insurance does not cover, including:

  • Extended and comprehensive visits
  • Secure messaging and between-visit support
  • Home healthcare and telehealth as clinically appropriate
  • Wellness, lifestyle, and spiritually grounded coaching
  • Proactive mental health check-ins
  • Navigation of care coordination, referrals, and follow-up
     

This structure allows us to provide consistent access, deeper care, and a more personalized experience.

 Membership fees are separate from insurance billing.


  • Patients with insurance will still have their insurance billed for medical services such as office visits, labs, and medications.


  • Your membership fees will be alloted to any copays, co-insurances, or deductibles you may have ensuring predictable pricing and no pop up fees.

$135.00 one time initiation fee

$75.00 one time initiation fee per person 

$120.00 per month for the first 3 people


$50.00 per month per additional family member

Family Without Insurance

$100.00 one time initiation fee per person

Hybrid Family Membership ( mixed-insurance and no insurance families)

This membership DOES NOT apply to those with Hip Nation/Sedera Health Insurance or Medicare

Yes! This membership is required for ALL patients who want to be seen at Sequence To Health. Our practice operates on a membership-based care model to allow for personalized, high-quality care and improved access.


The membership fee covers non-insurance services, including:

  • Care coordination
  • Enhanced access to the provider outside of scheduled visits
  • Multiple services in one visit (i.e. both primary care and mental health)
  • Wellness and spiritual coaching
  • Administrative and communication support
  • Extended visit availability
  • Home health and telehealth as needed

We still accept insurance and will bill your insurance for office visits; however, membership fees cover separate fees from insurance and are required for enrollment in the practice. 


No! For those with insurance, part of your monthly membership fee will go towards any copays, co-insurances, or deductibles you may have. This ensures you have simple, predictable payments for your healthcare. 


All monthly membership fees will be automatically billed on the 5th of the month, regardless of when the initiation fee was paid.


Memberships may be cancelled with written notice per our policy, but initiation fees are non-refundable.
